MRI Diagnosis of Mal ignant Meningioma

Abstract
Objective:To investigate MRI features of malignant meningioma,in order to improve diagnostic rate of the disease preoperatively.
Methods:18 cases with intact MRI materials proved operatively and pathologically were collected.
Results:Most tumors showed heterogeneous and low signal intensity on T1WI,(16/18 cases),and heterogeneous and high signal intensity on T2WI,(14/18cases).15 of 18 cases showed significant but inhomogeneous contrast enhancement.The tumors were irregular or lobulated in contour with all or part of the border ill - defined.
Conclusion:Malignant meningiomas do have characteristic MRI features besides in common with benign ones.

脑膜瘤是一种生长缓慢的肿瘤,占颅内肿瘤的15%~18%,其中绝大多数为良性,恶性者少见,占2%~10%。本文回顾性分析经手术病理证实且MR检查资料完整的恶性脑膜瘤18例,探讨其MR表现特征,提高其术前诊断率。
材料与方法
1.1 一般资料 病理证实的18例中,男8例,女10例。发病年龄22~69 岁,平均48.3岁。病史2月~5年不等。临床症状以头痛、肢体运动障碍为主,少数(4例)首发症状以视觉缺失或恶心呕吐为主。
1.2 成像技术 全部病例均平扫后行强化扫描。应用GE Vectra II 型0.5T超导型MR扫描仪。采用头部线圈,常规选择SE及FSE序列,T1WI (TR/TE 320 ms/15 ms),T2WI(TR/TE 3800 ms/100 ms),层厚6 mm,层间距2 mm,行横轴位及矢状位平扫。增强扫描,造影剂采用先灵公司产马根维显,剂量0.2 ml/kg,经肘静脉快速注入,行横轴面、冠状面T1WI (SE 序列TR/TE 340 ms/15 ms)扫描。
结 果
2.1 肿瘤发生部位及大小 18例中,15例为首诊恶性脑膜瘤,3例为复发性恶性脑膜瘤。发生部位依次为大脑凸面8例,镰旁4例,矢状窦旁3 例,嗅沟1例,蝶骨嵴1例,右侧脑室三角区1例。肿瘤大小2.4 cm×3.0 cm ×4.0 cm~9.2 cm ×6.0 cm ×8.3 cm,平均4.6 cm ×4.4 cm ×5.2 cm。 |
