螺旋CT和MRI在进展期胃癌术前分期中的价值

Abstract

  ObjectiveTo evaluate the diagnostic accuracy of spiral CT and magnetic resonance imaging for preoperative staging of advanced gastric carcinomas.

  MethodWith water fillingspiral CT and MRI examination was performed in 10 normal persons and 30 cases of advanced gastric neoplasma diagnosed by the operationSCT and MR dynamic enhanced scan was also applied. Spiral CT and MRI finding were compared with postoperative pathology.

  ResultsThe accuracy of T staging with SCT and MRI were 83.3%(25/30and 90.0%(27/30);The accuracy of SCT and MRI features for lymph node metastasis was 75.0 and 52.3%;The accuracy of SCT and MRI in M staging was 80 and 100.

  ConclusionMRI is superior to SCT in the evaluation of Tstaging and Mstaging. HoweverMRI has limitation in the evaluation of lymph node metastasis .

  胃癌是我国消化道最常见的恶性肿瘤之一,术前对胃癌做出正确、合理的分期对制订胃癌治疗方案及判断预后有着重要的意义。螺旋CT多期增强扫描的应用,大大提高了胃癌分期的准确性。随着MRI快速成像技术的发展,并由于其本身所固有的许多优点(多平面成像、高组织分辨力及无辐射),使MR对胃癌的术前分期准确性的判断有望提高,本研究旨在比较螺旋CTMRI对胃癌术前分期的影像学检查的价值。
