原发性喉淀粉样变的影像学分析(附九例报道)

ABSTRAC

Purpose: To improve the diagnosis and understanding of primary laryngeal amyloidosis by analyzing its imaging findings.   

Materials and Methods: 9 cases of primary laryngeal amyloidosis were confirmed pathologically, including 6 males and 3 females. 4 cases of CT exams and 5 cases of MR exams were performed  respectively.

Results: 5 of 9 cases demonstrated localized nodular or mass-like appearances; the laryngeal structures show diffuse thickening in 3 cases, causing airway stenosis or deformity, no obvious nodule or mass were present; 1 case demonstrated combined imaging findings, showing diffusely thickened laryngeal structures with localized nodular appearance. CT value of the lesions was 39.9-69Hu, and calcification was identified in one case. the signal intensity  is similar to muscle tissue on T1WI, and hypointense  to muscle on T2WI.

Conclusion: Imaging exam is very important in detecting the contour, type and extent of primary laryngeal amyloidosis£¬ but it still needs biopsy to establish the diagnosis.

原发性喉淀粉样变或称淀粉样瘤是一种少见病,是一种局部蛋白质代谢异常疾病。国内外有关淀粉样变影像学表现的文献报道较少[1]。本文收集了9例经手术证实的原发性喉淀粉样变,现报道如下,旨在提高对喉淀粉样变的认识。

 

一般资料

本组9例中男性6例,女性3例,年龄36-64(平均48.3岁)。9例在临床上均表现为声嘶,病程最短为2个月,最长11年,平均为23.4个月,有2例伴有喉异物感,3例伴呼吸不畅,1例刺激性干咳,1例反复咽痛、咽干。所有病例均在支撑喉镜下行肿物切除证实。

5例MR检查采用荷兰Philips公司Gyroscan 0.5T超导型磁共振成像设备,应用自旋回波(SE)序列,均采用表面线圈,T1WI:TR/TE为479-561/15-25ms,NSA(信号采集次数)为4,T2WI:TR/TE为1800-2226/80-90ms,NSA为2-8,层厚为4-6mm,间距为0.4-0.6mm,扫描矩阵为205×256,采集横断位及冠状或矢状图像,3例行Gd-DTPA增强扫描,剂量为0.1mmol/kg。4例CT检查采用Siemens ART型全身CT扫描机,层厚为3-5mm,层间距为3-5mm。
