Heart and coronary artery imaging with sixteen slice spiral CT: an initial study <?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />

ABSTRACT Objective: To evaluate the scan technique and primary clinical value of sixteen slice spiral CT scan in coronary artery and stent imaging. Methods: Plain and enhanced sixteen slice spiral CT imaging was performed in 69 cases, including 64 cases of suspected coronary heart disease and 5 cases of coronary stent. Calcium scoring with plain scan images and MPR, MIP and VRT reconstruction with enhanced scan images were made in all cases, among them 10 cases underwent conventional coronary angiography. We evaluated the demonstration and stenosis of coronary arteries, analyzed the factors which could influence the image quality. Additional MPR and MIP reconstruction of slices perpendicular to the stent and virtual endoscopy of stent were made in 7 stents of the 5 cases of coronary stent. We evaluate the patency of the stents. Result: Coronary calcium score could be got by plain scan with sixteen slice spiral CT. All of the first, most of the second and the third, part of the fourth subsegment branches could be shown isotropically with MP,IP and VRT reconstruction images of enhanced scan. MPR and MIP are better than VRT in the demonstration of small branches. In the evaluation of image quality with VRT images, 79.7% patients were in the first class, 10.1% were in the second class,10.1% were in the third class. The image quality is related to the contrast concentration in the root of the aorta,heart rate,with or without arrythmia and breath-hold ability of patients. Stenosis of coronary arteries were evaluated in 10 cases, which is comparable to that of the conventional coronary angiography in ten cases. 6 stens were evaluated to be patent, while 1 stent was not patent. Conclusion: Sixteen slice spiral CT scan of the cardiac coronary artery is a noninvasive,simple and good method. Coronary calcium score and coronary artery stenosis can be evaluated within one study. The patency of stents could also be evaluated. But more experience in more cases of comparative study should be obtained to prove the accuracy of the evaluation of stenosis of coronary arteries and patency of stents.
