Abstract
Objective:The upper airway (UA) dynamic imaging using Multi-slices Spiral Computed Tomography (MSCT)was applied to Obstructive Sleep Apnea Hypopnea (OSAHS) combined with multi-planar reconstruction technique (MPR).The cross-section areas in pharynx and the site of UA obstruction were calculated and estimated to investigate the value of MSCT in the application in the clinical diagnosis and treatment of OSAHS.
Methods:The UAs of 24 OSAHS patients and 12 controls were scanned by MSCT at the time of quiet stable respiration,deep inspiration, deep expiration, and Müller action. The cross-section areas at different pharynx levels were measured respectively under the four respiratory states. The obstruction locations were estimated. Correlation analysis was carried out between the MSCT and clinical data.
Results:The cross-sections of pharynx in OSAHS were smaller than those of the controls, especial at the soft palate and tongue levels. The cross-sections areas at the soft palate level under the quiet stable respiration and deep inspiration states have significant statistical difference among different severity of OSAHS and have negative correlation with AHI. There were also negative correlation between the cross-section areas at the naso-pharyngeal level and the neck circumference.
Conclusions:Dynamic scanning and calculation of UA with MSCT permit to observe the stenosis of UA effectively, and have reference value in guiding the clinical diagnosis and treatment.
